Updated Health and Safety Incident Report

The Health and Safety Committee has recently approved an updated version of the Occurrence Report. This report must filled out whenever there is a health or safety incident/accident on campus. Please ensure all parties are safe and out of danger prior to filling this out.

In the near future there will be a permanent location where this can be easily accessed. In the meantime the form can be accessed from here. You will need your Concordia Staff/Faculty/Student credentials to log in. (https://bit.ly/2A65K3Z)
